ABSTRACT

Osteoporotic fracture is the most major complication of osteoporosis. It is a significant global public health problem which continues to bring serious family and social burden. In order to improve the prevention and treatment of osteoporotic fractures, many countries have carried out new ways of fracture prevention and management, and achieved some results. Prevention mainly aims at those at high risk of fracture, and includes new health education on fracture and the promotion of fall prevention measures. For management, the "Fracture liaison services" mode is mainly applied to the fracture patients, with refined patient management to deal with the fractures that have occurred. This article summarizes the global prevalence of osteoporotic fractures, risk factors, and the current research progress in fracture prevention and management in China and the West.

ABSTRACT

Objective:To explore the efficacy and safety of ABO-incompatible (ABO-I) liver transplantation for hepatocellular carcinoma.Methods:Forty-four ABO-I liver transplantation recipients were matched with ABO-compatible (ABO-C) recipients by propensity score matching in a ratio of 1: 2. The cumulative overall survival (OS) rate, disease-free survival (DFS) rate and complications were compared between two groups.Results:Compared with ABO-C group, the levels of serum creatinine (sCr) were significantly higher in ABO-I group at Days 7 and 14 post-operation (89.1±36.9 vs 74.8±26.2 umol/L, P=0.001; 77.9±27.6 vs 67.6±18.6 umol/L, P=0.002). The incidence of hepatic arterial thrombosis (9.1% vs 1.1%, P=0.024), biliary complications (25.0% vs 8.0%, P=0.007), early allograft dysfunction (52.3% vs 31.8%, P<0.001) and acute kidney injury(68.1% vs 36.4%, P<0.001) also significantly spiked in ABO-I group. The postoperative cumulative OS, DFS and graft survival rate of ABO-C group were significantly higher than those of ABO-I group ( P<0.001). No inter-group difference existed in survival rate or complication incidence in accordance with the Hangzhou criteria. However, OS, DFS and graft survival rates of ABO-I group were significantly lower than those of ABO-C group ( P<0.001) and the incidence of hepatic artery thrombosis (6.7% vs 0.0%, P=0.043), biliary complications (30.0% vs 6.7%, P=0.003), early allograft dysfunction (53.3% vs 28.3%, P=0.020) and acute kidney injury (63.3% vs 28.3%, P<0.001) significantly rose exceeding the Hangzhou criteria. Conclusions:ABO-I liver transplantation does not affect the OS rate, graft survival rate and postoperative complications in accordance with the Hangzhou criteria. For HCC recipients exceeding the Hangzhou criteria, the prognosis of ABO-I liver transplantation is significantly inferior to that of ABO-C liver transplantation. Careful implementations and accurate evaluations should be performed for ABO-I liver transplantation. Patients exceeding the Hangzhou criteria may receive down-staging treatment so as to obtain transplantation opportunities and yield a better prognosis.

Objective@#To investigate the effectiveness and adverse effects of Cyberknife stereotactic body radiotherapy (SBRT) on liver metastases from PCa.@*METHODS@#From June 2009 to September 2016, we treated 20 cases of PCa liver metastases by Cyberknife SBRT, at a total dose of 36 (30-50) Gy, on 1-3 liver metastatic lesions, for 3-5 times, with a prescription isodose line of 70-92%. We assessed the therapeutic effect according to the modified Response Evaluation Criteria in Solid Tumors (mRECIST), calculated the survival and disease-control rates using the Kaplan-Meier method, and analyzed the adverse events based on the National Cancer Institute Common Terminology Criteria for Adverse Events-Version 4.0 (CTCAE 4.0).@*RESULTS@#Of all the cases treated, complete response (CR) was found in 8 (40.0%), partial response (PR) in 9 (45.0%), stable disease (SD) in 2 (10.0%), and progressive disease (PD) in 1 (5.0%), with a local control rate (CR+PR) of 85.0% and a disease-control rate (CR+PR+SD) of 95.0%. Among the 14 patients with elevated PSA, 10 (71.4%) showed a significant decrease after treatment. The median follow-up time was 17 months, the 1- and 2-year survival rates were 85.0% and 15.0%, respectively, and the median survival time of the 20 patients was 16.5 months (95% CI: 12.12-22.88). Cyberknife SBRT was well tolerated in all the patients, with only a few mild adverse events (mainly grades 1 and 2 but no 4 and 5) during the whole course of treatment.@*CONCLUSIONS@#Cyberknife SBRT is safe and effective in the treatment of PCa liver metastases, with a high local control rate, and capable of reducing the PSA level and raising the long-term survival rate of the patients.

ABSTRACT

Objective·To compile the osteoporosis prevention and control behavior scale for health care workers in community and evaluate its validity and reliability.Methods·In 20 standardized diagnosis and treatment pilot community health service centers for osteoporosis in Shanghai,400 health care workers were conveniently sampled to conduct the survey and 20 of them who were randomly sampled underwent retest 2 weeks later.The validity of the scale was evaluated by content validity and structural validity.The reliability of the scale was tested by Cronbach's αt coefficient,retest reliability and split-half reliability.Results·A total of 19 items were retained after project analysis.Scale-level content validity index/universal agreement (S-CVUUA)and scale-level CVI/average (S-CVI/Ave) were 0.944 and 0.992,respectively,after an entry with item-level CVI (I-CVI) less than 0.78 was deleted.Four common factors were extracted by exploratory factor analysis,and the cumulative variance contribution rate was 85.94%.Results of confirmatory factor analysis indicated that the model was consistent with the theoretical assumption,and the fitness indexes x2/df,CFI,GFI,and RMSEA were 3.323,0.971,0.902,and 0.076,respectively.The overall Cronbach's α coefficient of this scale was 0.978 and the Cronbach's αt coefficients of the four dimensions were all over 0.9.The intraclass correlation coefficient (ICC) of each item was greater than 0.7,and the ICC of the total scale was 0.654.The split-half reliability coefficient was 0.932.Conclusion·The osteoporosis prevention and control behavior scale has good reliability and validity and can be used to evaluate the implementation of osteoporosis prevention and control behavior of health care workers in community.

ABSTRACT

Objective To execute physical performance testing of Mobetron 2000 mobile intraoperative radiotherapy system.Methods Mobetron 2000 mobile intraoperative radiotherapy system had its indexes measured according to TG51 protocol such as percent depth dose (PDD),output dose,beam energy,field flatness and symmetry,X-ray pollution,dose linearity and repeatability.Three-dimensional water phantom was used for verification and acceptance when the measurement was executed,while the calimator end was tangent to the water surface and the nominal ource-skin distance was 50 cm.Results All the indexes of the system met the desired requirements except 12 MeV beam energy.Conclusion The system can be promoted clinically for safe,reliable and efficient treatment.

ABSTRACT

Taxol is one of the most potent anti-cancer agents, which is extracted from the plants of Taxus species. Isopentenyl diphosphate isomerase (IPI) catalyzes the reversible transformation between IPP and DMAPP, both of which are the general 5-carbon precursors for taxol biosynthesis. In the present study, a new gene encoding IPI was cloned from Taxus media (namely TmIPI with the GenBank Accession Number KP970677) for the first time. The full-length cDNA of TmIPI was 1 232 bps encoding a polypeptide with 233 amino acids, in which the conserved domain Nudix was found. Bioinformatic analysis indicated that the sequence of TmIPI was highly similar to those of other plant IPI proteins, and the phylogenetic analysis showed that there were two clades of plant IPI proteins, including IPIs of angiosperm plants and IPIs of gymnosperm plants. TmIPI belonged to the clade of gymnosperm plant IPIs, and this was consistent with the fact that Taxus media is a plant species of gymnosperm. Southern blotting analysis demonstrated that there was a gene family of IPI in Taxus media. Finally, functional verification was applied to identify the function of TmIPI. The results showed that biosynthesis of β-carotenoid was enhanced by overexpressing TmIPI in the engineered E. coli strain, and this suggested that TmIPI might be a key gene involved in isoprenoid/terpenoid biosynthesis.

ABSTRACT

Objective To explore influences of pre-tension and testing site on the radial compliance of vascular prostheses. Methods Three different textile vascular prosthesis samples (PET woven, PET/PPT woven and PET corrugate warp knitted) with the diameter of 8 mm were selected. Under various pre-tensions and testing sites, the pulsatile pressure and its corresponding radial diameter of these samples were measured by dynamic compliance testing instrument. The radial compliance was calculated according to the compliance formula in ISO 7198-1998. Results The radial compliance of all the three vascular prostheses increased along with the pre-tension increasing. When the pre-tension was 500~550 mN, all the radial compliance reached the maximum (1.03~1.72)%/100 mmHg (1 mmHg=0.133 kPa), and then began to decrease with pre-tension increasing. Furthermore, the radial compliance of corrugate vascular prostheses was slightly bigger at valleys than that at peaks, while the radial compliance at peaks was more stable than that at valleys. Conclusions The pre-tension and test site had vital effects on testing the radial compliance of vascular prostheses. Pre-tension of 500~550 mN should be right to apply to all the three kinds of textile vascular prosthesis at test. As for corrugate vascular prostheses, the radial compliance at peaks should be chosen as the test site to get more stable result and reliable data.

ABSTRACT

<p><b>OBJECTIVE</b>To evaluate the clinical efficacy of electroacupuncture(EA) for apoplectic urinary incontinence.</p><p><b>METHODS</b>Two hundred and four cases of apoplectic urinary incontinence were randomized into an EA group (136 cases) and an indwelling catheter group (68 cases). The EA was applied at Qugu (CV 2), Zhongji (CV 3), Shuidao (ST 28), Qihai (CV 6) and Guanyuan (CV 4), etc. in the EA group,5 times a week. Indwelling catheter was applied in the indwelling catheter group at intervals of 2-4 hours, and periodic bladder irrigation along with bladder rehabilitation training were also given. The efficacies were evaluated after 4 weeks of treatment. Before and after treatment, the urination diary (including the interval of urination, nocturia frequency, urination difficulty, urinary incontinence severity), bladder capacity, patients' satisfaction of the two groups were observed and the efficacy was evaluated.</p><p><b>RESULTS</b>The total effective rate was 96.2% (125/130) in the EA group, which was apparently superior to 87.5% (56/64) in the indwelling catheter group (P < 0.05); except for nocturia frequency in the indwelling catheter group, the total score and the subitem score in the urination diary were all improved significantly after treatment in both groups (all P < 0.001), which were more obvious in the EA group (P < 0.001, P < 0.05); the patients' satisfaction and bladder capacity were all improved significantly after treatment in both groups (all P < 0.001), which were more obvious in the EA group (both P < 0.001).</p><p><b>CONCLUSION</b>The EA has an obvious effect for apoplectic urinary incontinence in urinary incontinence alleviation and bladder capacity increase, which has better efficacy than indwelling catheter therapy.</p>

ABSTRACT

Objective To investigate the protective effect and associated mechanism of PNS in spinal cord hemisection injury.Methods fifty-five adult SD rats were randomly divided into three groups,sham group(n =5),spinal cord injury group(n =25),PNS group(n =25).The rats were evaluated in behavioral test with BBB score,pathology and immunohistochemistry at 1 d,3d,7d,14d,21d after the procedures.Results Motor recovery was significantly better in PNS group than the spinal cord injury group at 3d,7d,14d and 21d.Nissl staining showed less neuron necrosis and more integrated neural cells in morphology.cPLA2 expression was inhabited in PNS group,and less number of positive cells were found in the group.Conclusion PNS can inhibit the expression of cPLA2 after spinal cord injury,which may be one of the mechanisms of its effect on promoting motor recovery.

ABSTRACT

<p><b>BACKGROUND AND OBJECTIVE</b>CybeKnife is a newly developed technology in the field of stereotactic radiosurgery/radiotherapy (SRS/SRT). Compared with conventional SRS/SRT, there are many advantages for CyberKnife in terms of treating tumors that move with respiration, being real-time image-guidance, frameless, high accurateness, and so on. Recently, it has been used to treat different types of malignant carcinoma including intracranial and caudomedial tumors. This study was designed to evaluate the short-term efficacy and toxicity of the CyberKnife radiotherapy for locally advanced pancreatic cancer.</p><p><b>METHODS</b>A total of 20 patients with locally advanced (stage II-III) pancreatic cancer treated with CyberKnife were recruited between April 2009 and December 2009. Of 20 patients, 13 were with cancer located at the pancreatic head and 7 were located at the pancreatic body and tail. The planning target volume (PTV) was defined as gross tumor volume (GTV) plus 2-3 mm, and more than 95% PTV should be covered by 75% isodose surface. The median of PTV was 47 cm³ (26-64 cm³). The median total prescription dose was 40 Gy (32-55 Gy) at 3-6 fractions. During treatment delivery, X-Sight Spine Tracking System was used in 5 patients to track movement of the tumor. Other 15 patients were implanted fiducials in the tumors to track movement of the tumor and patient breathing patterns.</p><p><b>RESULTS</b>The median follow-up time was 7 months (3-11 months). All patients had finished the treatment and 19 were alive by the last follow-up. Slight fatigue was the most common complain. Evaluated by CT scan, 6 were complete response, 9 were partial response, 3 were stable disease, and 1 was progression; 1 was dead. There were 6 patients with grade I granulocytopenia, 7 with grade I nausea, and 5 with grade II vomiting.</p><p><b>CONCLUSIONS</b>The CyberKnife radiosurgery for the locally advanced pancreatic cancer shows a high rate of local control and minimal toxicity. Long-term follow-up is necessary to evaluate the survival and late toxicity.</p>

ABSTRACT

<p><b>OBJECTIVES</b>To investigate the intestinal microflora status related to ischemia/reperfusion (I/R) liver injury and explore the possible mechanism.</p><p><b>METHODS</b>Specific pathogen free grade Sprague-Dawley rats were randomized into three groups: Control group (n=8), sham group (n=6) and I/R group (n=10). Rats in the control group did not receive any treatment, rats in the I/R group were subjected to 20 min of liver ischemia, and rats in the sham group were only subjected to sham operation. Twenty-two hours later, the rats were sacrificed and liver enzymes and malondialdehyde (MDA), superoxide dismutase (SOD), serum endotoxin, intestinal bacterial count, intestinal mucosal histology, bacterial translocation to mesenteric lymph nodes, liver, spleen, and kidney were studied.</p><p><b>RESULTS</b>Ischemia/reperfusion increased liver enzymes, MDA, decreased SOD, and was associated with plasma endotoxin elevation in I/R group compared to those in the sham group. Intestinal Bifidobacterium and Lactobacillus decreased and intestinal Enterobacteria and Enterococci, bacterial translocation to kidney increased in the I/R group compared to the sham group. Intestinal microvilli were lost, disrupted and the interspace between cells became wider in the I/R group.</p><p><b>CONCLUSION</b>I/R liver injury may lead to disturbance of intestinal microflora and impairment of intestinal mucosal barrier function, which contributes to endotoxemia and bacterial translocation to kidney.</p>

ABSTRACT

Objective To explore the simplified method by examining the levels of luteinizing hormone(LH),follicle-stimulating hormone(FSH),estradid(E2) and stosterone(T) at different times in the stimutation test of luteinizing hormone-releasing hormone(LHRH).Methods Sixty patients with precocious puberty accepted LHRH stimutation test.The levels of E2,T,FSH,LH before injection and after 30,60,90 minutes were compaired.The levels of stimutation test of LHRH examined with ACS:180 chemiluminesence.Results There were 39 patients in the group of central precocious puberty(CPP).The levels of FSH and LH in CPP group significantly increased after LHRH 30 minutes injection.The ratio of LH/FSH was higher than 1.The peak level of LH was higher than 12 IU/L.There were 21 patients in group of peripheral precocious puberty(PPP).Compared with the results before injection,the levels of LH were similar to the results of 30,60 and 90 minutes after LHRH injection.Compared with the result before injection,the levels of E2 and T were similar to the result of 60 minutes after LHRH injection.The peak levels of LH and FSH in two groups all focused in 30,60 minutes.Conclusions LHRH stimutation test is mainly based on the peak level of LH and the ratio of LH/FSH,the test can be simplified to examine the levels the of LH and FSH before the test and 30,60 minutes after injection as a basis for the clinical diagnosis.
